All candidates are required to provide: At least three academic references that attest to the candidate s competence and potential for success in the profession. A copy of a current (i.e., within the past four weeks) photograph or video of the candidate. An official transcript from the appropriate academic institution showing successful completion of an accredited examination within the past four years. A list of courses completed and grades received. A certification letter from a recognized institution verifying the accredited nature of the course(s). Please note: A Candidate s transcript is considered acceptable when it was completed outside the University with a recognized institution of higher education. This document can be submitted upon request. The transcript can be on both official and unofficial forms depending on the requirements of the particular institution. If you need an electronic copy of an original transcript you must provide the official or unofficial copy. Please note that the official copy must be on official documents (e.g., college letters/report/registrations/registrant/transcripts) and must match the date of the date(s) on the unofficial transcript. Any other relevant documentation will be considered in your final decision to accept or deny a candidate for a license. If at any time during your examination you believe an applicant may have an impairment that would prohibit them from successfully completing their examination or from being licensed to practice, you are strongly encouraged to provide this information to the BOSH Medical Council Office for further consideration.
